Request for Medical Records
You will have to sign a release form in our office to obtain copies of your medical records. Your records will be copied and mailed to the address you specify. Please allow seven to 10 days for this to happen. There is a small fee for personal copies. If you have questions, call 751-3675, ext. 4.
